What to bring to an appointment
When attending your first consultation with Dr Freeman, please be sure to bring the following with you:
- Any scans or tests relating to the condition you want to discuss
- A referral from your local doctor is required if you wish to receive the Medicare rebate
- Medicare card and pensioner card, if applicable

Please provide us at least 48 hours notice if you need to reschedule your appointment, so that we are able to contact patients on our waiting list.
